[DAC] Open Studios printable registration form

Hello Everyone,

We have received several calls and e-mails regarding a glitch in the
printable version of the registration form.  The problem has been fixed, but
in case you are having incompatibility issues, I am attaching a Word.doc
version of the form here.  Also, because this problem was on our end--and
because the holiday week has caused people to scatter, the deadline for
registration has been extended to Monday, July 23rd.

You are still encouraged to register sooner rather than later in order to
assure your first choice of group site venue.  As a reminder, printed
registration forms should be mailed to :
